APPOINTMENTS

We see patients by appointment only. Appointments must be made in advance by directly calling our office. We make every attempt to see you at your scheduled time. However, we appreciate your patience if the doctor is late because of a hospital emergency. If you must cancel your appointment we require 24 hour notice to allow another patient to have your appointment time. A fee of $25 will be charged if cancellation is not received within 24 hours of your appointment. This fee cannot be billed to your insurance.

MEDICATIONS

Please promptly report to us any problems you experience with your medications. Please bring your medications or a list of your medications and how they are taken to each office visit. Medication refills can be ordered Monday through Friday. We are not available to refill your medications after hours or on weekends. Please allow 24 -48 hours for refills to be processed. (Refill request should come directly from your pharmacy).

PAYMENTS

We will file your office and hospital charges with your insurance carrier. Patient co-pays and deductibles are due at the time the services are rendered. If your insurance carrier does not pay within sixty days, you will be responsible for payment of your account. Our practice participates in the We Care Program. If you are uninsured and unable to pay, please contact our front desk to obtain contact information for the We Care Program.

DIAGNOSTIC TESTING

Diagnostic testing is frequently required. We order only the tests we regard to be important and necessary. The physician will review all test results. You will be notified only if the results are abnormal or if a change needs to be made in your care.

EMERGENCIES

When urgent problems occur during off-hours, weekends, or holidays, one of our physicians is still available. Please call the office telephone at 850-878-1171 for directions on how to contact the on call physician using a voice mail system.

QUESTIONS BY PHONE

Our nurses are well-trained to take information on your medical questions. Please call before 1:00 pm in order that a physician can be consulted in time for a response on the same day. If you have an emergency, the nurse will immediately contact the physician, and/or recommend a plan of action.
